My early training filed pregnancy under 'reproduction', a natural process the body was designed for. This tidy administrative category is a dangerous oversimplification, one that serves a medical system comfortable with dismissing women's lived experience. Pregnancy is a nine-month cardiovascular and autonomic stress test, and I've come to see it as the ultimate measure of a system's capacity. The symptoms you're told are 'normal' are direct readouts from the control room.
It's week 28. You've carried the laundry up one flight of stairs and now you're sitting on the edge of the bed, heart hammering against your ribs, a faint buzzing in your ears. You told your doctor about the dizziness, the headaches, the feeling that your own body is a foreign country. You were told it's 'just pregnancy'. Being told your reality is not real, while your body is screaming that it is, is a specific kind of psychological friction. The exhaustion you feel is compounded by the cellular weight of not being believed, a threat your nervous system takes personally.
Your blood vessels are managing a flood tide
During pregnancy, your total blood volume increases by up to 50%. This is a flood. Your heart has to pump harder and faster to move all this extra fluid through the same network of pipes—your arteries and veins. To cope, your vascular system, particularly the delicate inner lining called the endothelium, must become more flexible and accommodating. For years, I focused on the hormonal shifts of pregnancy, but I completely underestimated the raw physics of the problem.
Think of your circulatory system as a regional water authority suddenly ordered to handle a year's worth of rainfall in a single season. The endothelium is meant to release nitric oxide, a gas that signals the smooth muscle in the vessel walls to relax and expand, making way for the increased flow. This is an elegant, high-energy solution. When the endothelium is healthy and well-resourced—with enough of the amino acid L-arginine and protection from oxidative stress—the adaptation is seamless.
When the system cannot adapt fast enough, you get signals. If nitric oxide production falters, your vessels become less pliable. They stiffen. Swollen ankles are a direct report of fluid dynamics going awry, as pressure forces liquid out of the vessels and into surrounding tissues. Gestational hypertension is the audible alarm from vessel walls that have become resistant under unmanageable pressure. They are failing to relax. Your brain, which demands 20% of your oxygen and blood flow, is exquisitely sensitive to these inconsistencies. The brain fog, the inability to find the right word, the dull headaches—these symptoms signal a supply chain issue, a report from a high-demand organ that its resources are being compromised by faulty plumbing.
The placenta is a greedy new organ
The source of this immense demand is the placenta, a temporary organ built from scratch. The placenta operates as an aggressive, metabolically expensive, and parasitic entity. It exists for one reason: to secure resources for the foetus, often at your expense. It is the most demanding houseguest you will ever have.
The placenta is a vascular organ that literally invades your uterine lining, remodelling your spiral arteries to create low-resistance, high-flow conduits for blood. It hijacks your circulatory system to ensure a constant, non-negotiable supply of oxygen and nutrients. This hijacking of your system is a biological negotiation where the placenta holds all the cards.
This organ has its own endocrine agenda, pumping out hormones that manipulate your physiology. One of its star players is human placental lactogen (hPL), a hormone that actively makes your body's cells more resistant to insulin. This keeps more glucose circulating in your bloodstream, ready for the placenta to siphon off for the foetus. It’s a clever strategy for the foetus, but it forces your pancreas to work overtime producing more insulin to manage your own needs, setting the stage for gestational diabetes.
The metabolic cost is enormous. The fatigue of the first trimester is more than hormonal; it is the feeling of your body being strip-mined for resources like iron, folate, and choline to build this entirely new, power-hungry organ. If your own nutritional reserves are low, or if your system is already dealing with inflammation, the placenta’s demands can tip you from compensated to decompensated. Understanding the placenta as a separate, demanding actor is key to interpreting your body's signals correctly.
The autonomic nervous system is the site manager
Coordinating this massive, nine-month project is your autonomic nervous system (ANS). It is the lead engineer, the site manager, and the entire logistics department, constantly adjusting heart rate, blood pressure, and blood flow to keep the whole operation from collapsing. It has to keep you upright and functional while simultaneously building a whole new person from scratch. This is an incredible feat of regulation, running on a finite energy budget.
Your ANS draws from the same metabolic account you use for work deadlines, family obligations, and processing the ambient stress of modern life. If your baseline state is one of high alert—a state of sympathetic dominance—then pregnancy adds a colossal, non-negotiable line item to your physiological expenses. Your system has no spare capacity. The budget is blown.
This is the mechanism behind feeling 'tired but wired'. Your body is bone-deep exhausted, but your nervous system is stuck in the 'on' position, pumping out stress hormones like cortisol and adrenaline just to get through a normal day. It is running on emergency generators. For a long time, I thought the goal was to simply manage the stress. I now understand the task is to actively finance the rest your system needs to do its job. Running on generators is inefficient and produces a lot of metabolic 'exhaust' in the form of oxidative stress and inflammation, which further degrades the system.
When you report this experience—a racing heart, shallow breath, an inability to sleep despite fatigue—and are told it is 'normal', your brain registers this dismissal as a social threat. The brain's threat-detection system, particularly the anterior cingulate cortex, processes social rejection with the same circuitry it uses for physical pain. This isn't a metaphor. The dismissal from a person in authority is a neurological event that triggers a cascade of stress hormones, further depleting your already strained budget. They dismiss the symptom, escalate the state, and make the problem worse.
Common Questions
Why does my heart race so much, even when I'm resting?
A higher resting heart rate is an expected adaptation to the 50% increase in your blood volume; your heart simply has more work to do. However, frequent episodes of a racing heart (tachycardia) that feel jarring or come with dizziness signal that your autonomic nervous system is struggling to regulate the new normal. This suggests your sympathetic 'accelerator' is stuck on, while the parasympathetic 'brake'—governed by the vagus nerve—lacks the resources or 'tone' to bring you back to a calm baseline. The system has lost its regulatory flexibility and is over-relying on adrenaline to manage moment-to-moment demands.
What is actually causing the headaches and dizziness in my third trimester?
While many factors can contribute, headaches and dizziness are often classic symptoms of blood pressure instability. Your entire vascular system is working overtime to manage the tidal wave of extra blood. When you stand up, your ANS is supposed to instantly constrict blood vessels in your lower body to push blood up to your brain. When the system is overloaded, this response can be sluggish, causing a momentary drop in blood pressure to the brain. That's the dizziness you feel (orthostatic hypotension). The headaches are a sign of fluctuating pressure and vascular tension, a direct signal from your vascular system that it is struggling to maintain equilibrium.
Can my stress at work really make my physical pregnancy symptoms worse?
Yes, unequivocally. Your nervous system has only one budget for stress, whether the source is physical, psychological, or emotional. The demand on your capacity is cumulative. The physical load of building a placenta and the psychological load of a toxic workplace draw from the same finite pool of metabolic and regulatory resources. A stressful meeting triggers the same adrenaline and cortisol response as a drop in blood sugar, adding to the total allostatic load—the cumulative wear and tear—on your system. Stress is a physical event, and it directly degrades your cardiovascular function and autonomic stability.
Why is the fatigue so different in the first and third trimesters?
The fatigue feels different because the primary causes are different. First-trimester fatigue is overwhelmingly metabolic and hormonal. It’s driven by the monumental energy cost of building the placenta and the powerful sedative effects of soaring progesterone levels. Third-trimester fatigue has a much larger mechanical component. It’s the physical weight of the baby restricting lung capacity, the 24/7 strain on your heart and blood vessels, the pressure on your hips and spine, and the sheer cumulative depletion of nine months of running a biological factory.
