    Context

After two years of post-pandemic overflow, the clinicians were running 32 patient hours a week and showing classic compassion-fatigue markers. Two had given notice. Leadership wanted to fix the rota; we argued the rota wasn't the problem.

    The pattern

Clinicians were absorbing patient dysregulation all day with no protocol to discharge it. They were leaving work carrying the day in their bodies — and bringing it back the next morning.

    What we did

    • •Group Regulation L1 cohort (8 weeks) for the full clinical team
    • •Between-patient anchor (90 seconds) embedded into the EMR workflow
    • •Monthly group debrief replacing one of the existing supervision slots
    • •Leadership coaching for the two senior clinicians to model the protocol

"We thought we needed more staff. We needed a nervous-system protocol. The rota didn't change — the way our team metabolised the work did."
