What the evidence actually supports
Evidence reviewed: 2026-10-21
Semaglutide & Tirzepatide
These medicines have substantial human evidence for defined metabolic indications. Approval belongs to a product, indication and country—not to every vial bearing the molecule's name. Appetite effects are not proof of nervous-system regulation or dorsal-vagal shutdown.
Protect nutrition, strength and hydration with an individual plan. An Anchor, supplement bundle or fixed fasting window is not a medical prerequisite. Obesity treatment may be long term; stopping commonly leads to regain, and a taper is not proven to prevent it. Do not change prescribed treatment based on wearable readiness alone.
New formulations and the metabolic pipeline
Oral Wegovy received US FDA approval on 22 December 2025. In OASIS 4 (307 adults without diabetes), mean body-weight change at 64 weeks was −13.6% versus −2.2% with placebo in the treatment-policy analysis. This is an oral formulation, not proof that every semaglutide product is interchangeable. Wegovy injection also gained a US accelerated MASH indication in August 2025 for noncirrhotic disease with F2–F3 fibrosis; that is not a liver-detox or longevity claim.
CagriSema has published phase 3 evidence: REDEFINE 1 reported −20.4% versus −3.0% at 68 weeks in the treatment-policy analysis. Retatrutide's published phase 2 trial reported −24.2% versus −2.1% at 48 weeks, a secondary time point. These are separate trials, not a head-to-head ranking. No approval was verified here for either pipeline agent; trial success and commercial release are different events. Current Portuguese supply and reimbursement remain unverified.
Semaglutide: the eye-safety update
On 6 June 2025, EMA concluded that NAION, an optic-nerve condition, is a very rare adverse effect of semaglutide. Observational estimates in adults with type 2 diabetes corresponded to about one additional case per 10,000 person-years; that estimate is not a risk calculation for every individual.
Sudden vision loss or rapidly worsening eyesight requires urgent medical assessment. This warning is specific to semaglutide, not established as an identical risk for all peptides or incretin medicines. The clinician handles treatment decisions.
